Office-based and OTP-delivered MOUD remains the backbone of opioid use disorder treatment in the US and globally. The session covers long-acting injectable buprenorphine formulations (Brixadi, Sublocade), real-world diversion and adherence data, methadone take-home flexibility after the 2024 SAMHSA permanence rule, integration with primary care and federally qualified health centres, methadone in pregnancy and breastfeeding, and the regulatory landscape post X-waiver elimination. Discussion addresses pharmacy access challenges, mobile MAT vans, telehealth-initiated buprenorphine after the 2025 DEA flexibility extension, and the global access debate as buprenorphine generics become more widely available outside high-income countries.
